Fill out treatment forms
A number of forms are available in the EHR module to help EMS personnel capture additional information about specific situations, such as a trauma, a burn, an intubation, and so forth. Your ESO Suite administrator can choose which forms to enable for your agency, depending on its data collection needs.

The buttons that appear on the Forms tab are defined in the Admin module, under EHR > Forms Tab > Forms.

Example: In the EHR module, in the Forms tab, you may need to display buttons for the Advanced Airway, Burns, FAST Stroke Scale, and many others. Your ESO Suite administrator must enable these forms in the Admin module, under EHR > Forms Tab > Forms, for these forms to appear in the EHR module.

If no forms appear, or if the Forms tab does not list forms that meets your needs, contact your ESO Suite administrator and ask them to enable the necessary forms. You will need to work closely with your administrator to create and maintain these lists, as your department's needs evolve over time.
