Mobile integrated healthcare - visit

A mobile integrated healthcare visit occurs when a provider or program representative visits the patient at their home, or when the patient goes to a facility or provider at their place of business. Home visits are appropriate for patients with a high risk of readmission, or who are frequent emergency medical service users. Visits can extend the reach of a medical facility to meet people where they are and help change behaviors, understand how the patient travels to primary care, assess the patient's diet, and see "what’s really going on in the home," not necessarily what the patient tells the doctor. Home visits may also include a social worker, who is able to further tailor their care plan or make additional referrals as linking individual patients to needed resources and identifying gaps in care.

  1. At the top of the page, click the Forms tab.

    The contents of the Forms tab appear, listing all the forms your ESO Suite administrator has enabled in the Admin module.

  2. Click Mobile Integrated Healthcare - Visit.

    The Mobile Integrated Healthcare - Visit dialog box appears.

  3. Click OK.

    The dialog box closes, and a green triangle appears in the upper right corner of the form button, to indicate that data exists in this form.
